How to Create a Custom Form
Premium and Premium Plus accounts can create custom forms to include in employment applications.
From the Home Page, select Custom Forms to open the Custom Form Builder.
Select the Custom Forms tab.
Select Create Form in the upper-right corner.
Enter a name for your custom form.
Select Create Form to finish.
